The Transitions Clinic in New Haven is a medical home for people in the New Haven area who have recently been released from prison and have a chronic medical condition.
The Transitions Clinic believes that people with a history of incarceration:- Have a right to confidential medical services
- Have the capacity to make decisions about their own healthcare
- Should be treated with respect
- Should have access to timely, tailored and effective healthcare services
