TCN California Learning Collaborative Accepting Applications!
In partnership with the California Health Care Foundation and The California Wellness Foundation, Transitions Clinic Network (TCN) is accepting applications from primary care clinics interested in implementing the TCN model of care through our California Learning Collaborative. Sites will participate in a year-long learning collaborative to implement a TCN Program to improve the health and reentry outcomes of individuals with chronic health conditions transitioning from incarceration. Selected sites will receive a year of structured technical assistance and training as well as funds to support participation and program implementation.
The TCN program is an evidenced-based model of care that reduces ED visits, hospitalizations and parole and probation violations for patients transitioning from incarceration. This model aligns with CalAIM’s Enhanced Care Management benefit. This is a unique opportunity for clinics to play a role in not just improving patients’ health, but improving communities’ health by fostering healthy reintegration and reducing recidivism.
