Program Implementation

TCN members receive comprehensive technical assistance and training on all aspects of Transition Clinic Program implementation, including needs assessment, culturally competent service development, outreach planning, and CHW integration. Through this support, member clinics have been able to tailor their existing services to care for chronically-ill patients returning from incarceration.
